Print out the Security Gift Transfer Form below.  The top portion has the information that you will need to give to your broker to transfer the shares.  The bottom of the form must be sent to Orchard Alliance so that they can track the shares in their Merrill Lynch account.  It can be sent by email, fax or mail.  It works best if you notify Orchard Alliance before the shares are transferred so they can watch for the arrival of the shares and have them sold right away.
